South Valley Neighborhood Partnership Arvin/Lamont/Weedpatch Collaborative

The South Valley Neighborhood Partnership (Arvin/Lamont/Weedpatch) Collaborative is the collaborative serving the communities of Arvin, Lamont, and Weedpatch. Through the collaborative, all organizations and agencies that serve these communities and our families are invited to come to the table and work with the other partners to make the services we provide to the community more effective and efficient.

SVNP meetings are held the first Thursday of each month from 10:00 to 11:30am at various locations throughout the community. To get more information or find the location of the next collaborative meeting, contact Janet at (661) 845-2724 or jhernandez@lesd.us.

Multi-Disciplinary Team Meetings

The SVNP hosts a Multi-Disciplinary Team Meetings when needed. This is a meeting for staff who work directly with families in a case management or family support role. At the Multi-Disciplinary Team Meeting, staff from multiple agencies discuss and strategize to improve the family's outcome and ensure that family support workers are providing the best services possible and coordinating services to ensure that they are efficient. 

When needed, these meetings are held on the first Thursday of each month from 1:30 to 3:30 pm at the Family Resource & Learning Center, 7915 Burgundy Avenue in Lamont. 

Family support workers can refer a family to the Multi-Disciplinary Team meeting by completing the Referral Form (click here to be redirected to the referral form).

As a reminder for family support workers, if you refer a family to the Multi-Disciplinary Team meeting, you should ensure that you have met your confidentiality and other requirements. Below is a sample form that has been used by agencies to meet these.
