Refer a Student or Family for Services

Please complete the form below to refer a student or family for services provided by the Family Success Center. Once you complete the form a Family Advocate will follow up with the family in 2-5 days depending on the urgency of the issue. You should expect a response to your referral in 2-4 weeks. If you have any questions, please contact the Family Advocate for your school site, or Janet Hernandez at jhernandez@lesd.us.
